You may reach them directly at 02 9382 7111. *Consultation Fees: A. A consultation fee is payable for every visit to our ophthalmology practice. B. The consultation fee covers a single assessment and discussion of your eye health concerns with our ophthalmologist. It excludes tests, procedures, and other non-related fees. *Procedure Fees: A. Each surgical procedure or treatment incurs separate fees. B. All fees and quotes for procedures refers to unilateral or one eye only C. Procedure fees cover the cost of materials, equipment, and professional expertise required to perform the procedure accurately. D. Excludes Hospital, Disposables, Anaesthetists, Hospital Assistant or Pharmacy fees E. Procedure fees exclude fees related to post-operative reviews, follow-ups, and additional tests. F. Post-operative reviews, follow-ups, and additional tests may incur separate charges as determined by the ophthalmologist based on your ongoing care needs. *Test Fees: A. Each diagnostic test or procedure incurs separate fees. B. Test fees cover the cost of materials, equipment, and professional expertise required to perform the test accurately. C. The necessity of tests will be determined by the ophthalmologist based on your specific eye health needs. *Payment: A. Payment for consultation fees and any applicable test fees is due at the time of service. B. We accept cash, credit/debit cards, excluding Diners and AMEX. C. Failure to make payment at the time of service may result in additional administrative charges. *Insurance: A. Health insurance will usually not cover any in-room treatments/procedures, consultations, and tests. Please enquire directly with your health fund provider for more details. B. Patients are responsible for any deductibles, co-payments, or non-covered services as determined by their insurance plan. *Fee Schedule: A. Our fee schedule for consultation and test fees are available upon request. B. We reserve the right to adjust our fee schedule at any time, with notice provided to patients when possible. *Medicare Rebate: A. We endeavour to send your claim directly to Medicare after full payment is received. It is the patient’s responsibility to claim directly from Medicare for their rebate should the online claiming portal/system fail. B. It is the patient's responsibility to ensure Medicare details, referrals are up to date and current. C. Medicare rebates only apply to some tests, procedures, and consultations. Please inquire with the clinical team to determine if the test you are having is Medicare rebatable or not. D. All our fees will incur a GAP. E. Should you not have a current and valid referral from your GP or Optometrist you will not receive Medicare rebate *Cancellation Policy: A. If you need to cancel or reschedule your appointment, please notify us at your earliest convenience and preferably at least 72 hours in advance. B. Failure to provide sufficient notice may result in a cancellation fee. *Financial Assistance: A. We understand that medical expenses can be a burden for some patients. B. Financial assistance or payment plans may be available for qualifying patients.
